Virginia Wrestling Media Relations Questionnaire

This is NOT a recruiting form and DOES NOT go to coaches.

This form will serve as a permanent record in the Athletics Media Relations office, and will be used for publicity purposes (media guide, website, etc.). Please be as complete and accurate as you can.

Please contact Bill Bunting (bunting@virginia.edu) with any questions or concerns.

Ex. Mike Smith instead of Michael Smith
Feet & Inches ex. 6-0, 5-11 etc.
Please include City, State (and country if outside U.S.)
Ex. John Smith & Mary Smith
Ex. Charlottesville High School (2000-2004)
Please list sport, letters won, statistics, team captain, all-city, all-county (specify name), all-conference (please specify names - NO INITIALS), all-state, etc. Also please list any records you set, team honors such as MVP, scholar-athlete, etc.)
Please provide previous school and years attended
If you have transferred from another institution please include the name of the school along with any other awards/honors/stats that may be relevant
(Big Brother/Big Sister, any volunteer work, initiatives/fundraisers you've worked closely with, church groups, athletic camps you worked, anything!) Please be specific because we won’t necessarily know what you’re talking about.
Ex. John - 13 years old Sally - 8 years old (if none, type none)
Please include relation to you (mother, uncle, brother, etc.), what sport and where they played if known.
